This letter is intended to explain our billing policy for patients who have insurance plans in which we participate and those who have plans in which we are not in the network of participating providers.

If we participate with your plan then, for your convenience,  we will bill the insurance company for the services provided. You will only be billed if the services are not covered by your health plan. Services may not be covered if a required preauthorization or pre certification is not provided or if the claim has inaccurate information. Therefore we ask that you review your insurance information and provide your card to our receptionist when you arrive for your test so she can assure that our information is up to date.

If we are not a participating provider with your insurance plan, we are happy to accept assignment provided that you have a plan which allows you to see physicians who are “out of network” and you have met your out of network deductible. Many PPO and POS plans will pay 70-100% of the customary fee for an imaging service that you have with an “out of network” provider. You may have an annual deductible, which is the amount you have to pay each year. You are responsible for your deductible and so this amount is billable. If you have met your deductible and you have out of network benefits then we can submit the claim for your exam.  We will ask that you assign those benefits to us. If you have not met the deductible, we ask that you pay your bill prior to leaving the office. We will then submit the claim to the insurance company for your convenience and the insurance proceeds will go directly to you. Please feel free to ask to speak to the billing manager if you have any question or if you need to make a special arrangement. Our priority is to make you feel comfortable and allow the doctors to provide you with the highest quality care in a personal environment.
